A Wilmington front door. A Claymont hub. A statewide pathway.

Participants begin at the Wilmington onboarding center, travel to the Claymont hub for programming and coordinated care, and move through transitional housing toward a lasting home in the communities they are from.

The Route

From first contact to a permanent address.

Each step is a real location. Transportation between them is run by the organization.

Wilmington

The onboarding center on Lea Boulevard

Registration, showers and laundry, assessment and benefits screening. The front door, inside the city.

Every day, about seven miles

Transportation the organization runs itself

Vehicles run both directions daily at no cost. A mobile medical and dental unit is planned for people not yet ready to travel.

Claymont

The campus hub

Workforce and life skills, financial and digital literacy, a teaching kitchen, and clinical care as staffing comes online.

Up to twenty-four months

Transitional housing

Participants move in as units come online. Length of stay is a ceiling, not a plan.

The destination

A sustainable destination, set by the pathway

Exit into the destination defined for that pathway. Alumni support continues, including a route back onto staff.

The same pathway extended south as capacity and partners allow. Not operating today.

Harmony Health does not own or develop real estate.

How the space is held

Housing insecurity is a social driver of health, which makes the buildings our programs run in a piece of healthcare infrastructure. Harmony Health secures the space it needs through long-term lease from privately held owners. That lease is what gives us the right to direct what happens inside: who is served, what services are delivered, and how care is coordinated.
