Care, programming and place are designed together and held under one accountability, so a person is never handed a bed in one place and left to find health, work and a future somewhere else.

Six Parts

One system, not six services.

Whole-Person Care

Medical, dental, and behavioral-health services are planned to be delivered by MaxWellness under agreement as programs activate, with services billed through each participant's applicable coverage, so health does not depend on getting across the county.

Day Programming

Structured day services at the hub for adults with intellectual and developmental disabilities and other participants, so people live in the community and come in for programming rather than being placed into an institutional setting.

Workforce Development

The planned MaxWellness Healthcare Academy will prepare Delaware residents for healthcare careers and help staff the same system its residents rely on.

Care Coordination and Social Drivers of Health

We coordinate the conditions outside the clinic, food, benefits, transportation, and connection, that determine whether care and stability actually last.

Housing and Smart Health Homes

Safe, stable, supportive housing secured by lease, including smart health homes out in Delaware neighborhoods, delivered in phases and aligned with state and community needs.

Transportation

Vehicles connect the spokes to the hub, bringing people in for day programming and workforce training and taking them home again, because getting a person to the service is part of delivering it.

The Pathway to Independence

Up to twenty-four months, with a destination.

Beginning the day a person is engaged on the street and ending in the most independent setting a person can sustain. A time-bound arc with a destination.

Weeks 1 to 4

Engage

Outreach and transport to the onboarding center. Showers, laundry, oral hygiene. Registration and assessment with a Success Coordinator. Benefits navigation begins on day one.

Months 1 to 3

Stabilize

Daily program attendance. Medical, dental and behavioral health homes established. An individual pathway plan built and agreed with the participant.

Months 3 to 18

Build

Transitional housing as units come online. Workforce training, credentials, financial and digital literacy, and employment placement.

Months 18 to 24

Transition

Independence, at the level this person can sustain. Alumni support, peer mentorship, and a route back onto staff.

How It Runs

How the pathway operates.

Operation Pathways, the daily cycle

Vehicles deploy at dawn. Participants arrive at the onboarding center for hygiene and registration before paperwork, then travel to the hub for workshops, instruction and clinical appointments. Lunch is provided. Everyone returns each evening. Day services run before the buildings are finished, so people begin the process while transitional units are still in preparation.

Why benefits navigation starts on day one

Benefits navigation begins on day one. Staff help participants apply separately for health coverage, income supports, and housing assistance for which they may qualify. Each has its own rules, paperwork and timeline, so this is staffed as a discipline rather than treated as a task.

What we measure

The share of participants who reach the destination defined for their pathway within twenty-four months, reported quarterly, published in full, including results that move the wrong way. Independent evaluation, with the comparison-group method stated before the first participant enrolls. Continuous measurement on a fixed cadence rather than an annual snapshot.
